Residential Services
Program Overview
Paladin’s Residential Habilitation Services support adults with disabilities in living safely and successfully, either in their own home or in a small, supportive living setting. Guided by each person’s Individualized Support Plan (ISP), our team provides consistent, person-centered support that promotes health, independence, and quality of life.
Services may include help with daily routines like personal care, meals, shopping, errands, household chores, leisure activities, and transportation (excluding transportation covered under the Medicaid State Plan), as well as coordination of medical and non-medical care such as appointments, medications, and health planning. Our staff also work closely with each person to implement ISP goals, Behavior Support Plans, and Risk Plans, ensuring supports are proactive, safe, and tailored to individual needs.
